Required Documents - Passport Issued After 16th Birthday
Please follow the instructions below if all of the following apply:

Your most recent passport was issued after your 16th birthday.

You are able to present your most recent passport with the application.

Your current name is the same as on your most recent passport, or you can provide legal documentation that your name has been changed.
Application Requirements:

Complete sections 1 through 19 on the application, using block letters and black ink. All fields are mandatory.

Item 5 – Social Security Number: You must provide this information, as applicants who fail to provide a valid number are subject to a $500 fine by the IRS. If you have never had a social security number, please enter zeros in the field. If you have a social security number but cannot remember it, please call the ACS office.

Item 11 – Mailing Address: Complete this item with a Czech Republic address that you use for work, home or temporary residence.

Item 12 – Permanent Address: Complete this item with the address you consider to be your permanent location.

Your most recent passport.

Photo. One 5×5 centimeter (2×2 inch) color photos is required, displaying your full face with a light background, no head cover and no glasses.

Evidence of your name change, if applicable. If you have changed your name due to marriage or other event, proper legal documentation identifying the change is required.

Parental consent may be required for security reasons for minors under age 16.

You are age 16 or older.

Your most recent passport was issued before your 16th birthday and / or more than 15 years ago.

You are able to present your most recent passport with the application.

Item 23 – Oath and Signature: Please do NOT sign the DS-11 form until you are in the presence of the Consular Officer at the Embassy. You must read the “Acts or Conditions” on page 4 of the application before proceeding. If any mentioned acts or conditions have been performed by or apply to you, cross out only the applicable portion(s) and submit a signed statement explaining the circumstances.

Child must be present. All children under the age of 16 are required to appear at the Embassy for all passport services.

Child’s most recent passport. If the child was a baby in the last passport photo, be prepared to provide an age progression of photos showing the child’s development since the last passport.

Parental consent is required for security reasons. Both parents must authorize all passport services for children under age 16.

Proof of each parent’s identity. A photo ID, such as your current passport, driver’s license or Czech identification, is required.

Proof of parentage. The child’s original birth certificate or similar document must be presented, listing both parents.

If one parent is absent, the applying parent must present the following:

Form DS-3053 Statement of Consent: Issuance of a Passport to a Minor Under 16 completed by the absent parent.

A signed and notarized copy of the absent parent’s passport. OR
-
A document identifying his/her authority to apply alone
-
Proof of outstanding family circumstances.
